Green Chiropractic Clinic is a growing four-year-old chiropractic practice founded by Dr. Jacob Sahourieh, DC. The practice offers a variety of wellness services, from chiropractic care for auto accident victims, acute injuries and massage therapy, to custom foot orthotics and nutritional services. Dr. Sahourieh and his staff use Loop as a questionnaire to streamline collecting health information from new patients as well as maintaining patient experience records to constantly improve quality and consistency of service.
When new patients arrive for their first visit they are directed by an assistant at the reception desk to a special kiosk to fill out Green Chiro’s new patient health questionnaire using an iPad. These health questions must be answered before meeting with a doctor.

* Streamlines new patient intake process * Meets patient privacy requirements * Offers branding & logo options New Patient Intake Efficiency Green Chiro wanted to collect new patient health information electronically to streamline the new patient intake process and to eliminate the cost and potential errors of a staff person manually entering data from a printed questionnaire. The app also needed to be very easy to use for new patients. Loop met these needs and more with the ability to download new patient information as an Excel document for fast and simple upload to their customer database. Searchable Records Loop has also been successful at allowing for quick and searchable patient visit records, especially with regard to subpoenas for medical records. Rather than having to search though paper sign-in sheets, staff can easily search email folders with keywords to either make confirmation of an appointment or use as proof that no appointment took place. App Privacy & Security Answers submitted by new patients remain private through the app’s optional “Lock” feature, which is manually triggered and prevents anyone from accidentally seeing or deleting someone’s questionnaire information. When the next new patient arrives, the account’s password is entered by staff to unlock the app. Additionally, all app data is saved and secured in real-time by Loop.
